Agnikarma is a thermal para-surgical procedure, in which “AGNI” is used
for intentional burning as therapeutic purpose. It is indicated for
various Vata- Kapha disorders.
Agnikarma has been explained as one among the anushastras. Almost all
ayurvedic classics have described the use of agnikarma in curing
different disorders as pradhanakarma and in some disorders as
paschathkarma to cure the complications. Its importance lies in its
action, because of its ability to cure those diseases also, which can’t
be cured by the bheshaja, shastra and ksharakarma. In this paper an
insight view on agnikarma has been compiled and presented
systematically.
Introduction:
The
word Agnikarma is made up of two words i.e., Agni and Karma. Agni, the
musculine word derived from the root agigatau agyati agnayamna
prapyanti. i.e., it gives the rebirth, is utilised to cure and prevent
the diseases. Agni is the important one among the pancha mahabhutas. The
Agni, in the form of bhootagni, jatharagni and dhatwagni is the life
for the creatures. On the basis of the loka – purusha sadharmya, the
Agni that exists in the body, possesses the same properties as that of
Agni in the world. So any procedure that involves the Agni directly or
indirectly is considered under agnikarma.
The
agnikarma is also known by different names like dahanakarma,
dahakarma, dagdhakarma, jalanakarma and pachanakarma, finds its role in
the vataja and kaphaja vyadhi involving the dhatu that are predominant
with pruthvi and ap mahabhutha. Mere utilization of agnikarma, which
is counted in anushastra, upayantra and shashti upakrama, provides
Shalyatantra the unique position among the eight faculties of Ayurveda.
Agnikarma can be utilised – As preventive measure, as curative
measure, as postoperative procedure, and as haemostatic measure.
Importance of Agnikarma:
Agnikarma
does not allow the diseases to reoccur, once they are treated by it.
Agnikarma is superior to kshara by means of its action. Agnikarma is
always utilised as the ultimate measure among the yantra, shastra,
anushastra, kshara etc., because of its ability to cure those diseases
also, which can’t be cured by the bheshaja, shastra and ksharakarma.
Agnikarma is the ultimate measure for the haemostasis among the four
raktasthambhana measures.
Indication of AngiKarma:
- Twak
– sira – mamsa – snayu – sandhi and astigata vata; kathina, uchrita
suptamamsa; vrana; granti; arsha; bhagandara; arbuda; apachi; shleepada;
Charmakeela; tilakalaka; antravrudhi; sirachedana; nadivrana and
shonita atipravrutti.
- All
the diseases, which are caused by vata and kapha dosha, involving the
sthayi dhatus like mamsa, meda, asthi, majja and upadhatu like sira,
snayu and twak that are pruthvi and ap mahabhutha pradhana, should be
treated with agnikarma on the basis of vipareeta chikitsa.
- In Vatajadisease like gridrace, vatakantaka etc.
Contra Indication for agnikarma
- Unsuitable in PittaPrakruti, Sharada and greeshma rutu, Bala and vrudha avasta.
- Unsuitable in persons like Durbala, bheeru.
- Unsuitable
conditions like Garbhini, vishadayukta, Antahashonita, rakta pitta,
anudhruta shalya, bhinnakoshta, anekavranapeedita, aswedya like pandu,
pramehi,trashnapedeta etc.
IMPORTANCE OF AGNIKARMA
- It is a highly potential procedure than the bheshaja, Shastra & ksharakarma.
- It is very effective to terminate the chance of recurrence of the disease.
- No infection.
- No secondary complication if done properly.
INDICATIONS
- Painful conditions of musculoskeletal disorders.
- Hard, elevated and desensitized vranas/ulcer.
- Granthi, Arsas, Bhagandara, Arbuda, Sleepada, Charmakeela and Antravridhi.
- Severe bleeding condition.
CONTRAINDICATIONS
- Pitta Prakruti
- Antah shonita (Internal bleeding)
- Bhinna koshtha (Ruptured viscera)
- Nuddhrita shalya (in presence of foreign body)
- Durbala (emaciated), Bala (children), Vriddha (old aged), Bheeru (coward), Aneka vrana peedita (with multiple wounds)
INFRASTRUCTURE FACILITIES
Manpower: A trained doctor and technician
Space
and requirements: 1 small sized room (10x 10) with proper light. It can
be done in OP level also. Chair or stool- 1, Examination table/ Droni-
1.
MATERIALS REQUIRED
- Shalaka
- Permanent Marker
- Gas stove, lighter
- Surgical spirit or Murivenna, Gloves
- Cotton gauze
- Haridra Churna (turmeric), Aloe vera pulp
PREOPERATIVE
- Educate the patient
- Obtain informed consent
- Ask the patient to satisfy natural urges
- Check vitals
PROCEDURE
- The patient should be allowed to be in a comfortable position, exposing the site of the problem.
- Keep Shalaka in the gas stove for heating.
- Wear surgical gloves.
- Clean the site with the surgical spirit or Murivenna.
- Mark the tenderest points with a marker.
- Ensure Shalaka to be red hot.
- Hold
red hot Shalaka strongly in one hand and touch in the marked points for
2-3 seconds only (depends on the site and tenderness).
POST OPERATIVE
- Keep Shalaka in specified the place.
- Apply Aloe vera pulp over the Agnikarma site. Wait for a while and then apply Haridra churna with cotton over there.

>> Digest Aama and achieve niramavastha >> Pacifies Vata
& Kapha
- Agnikarma produces a coagulative necrosis on the
surface layers of skin which causes self-healing. There is no need of
post-procedural medication locally or orally.
EFFECTS OF AGNIKARMA
- Increases metabolism
- Increases blood circulation
- Decreases pain
- Exciting/stimulating nerves
- Relaxation to muscles
- Decreases infection
- Decreases joint stiffness and inflammation.
APPLICATION & DURATION
- It depends up on site & disease
- Usually 2 to 5 seconds & more than 5 seconds in corn, callosity etc.
Effect of Agnikarma on
mamsa dhatu will reaches into sira, snayu, asthi & sandhi.
TIME / SEASON
All seasons except Greeshma & Sharat Ritu. In the case of emergency, it can be done in any season with special precaution.
PRECAUTIONS
- Agnikarma should not be done empty stomach. It is better to take Pichchila anna before Agnikarma.
- Should be aware of vital parts of the body, season, vyadhi dhosha avastha.
- Care should be taken while heating Shalaka and placing it in specified place after Agnikarma.
COMPLICATIONS AND MANAGEMENT
No specific complication if done properly.
CONCLUSION
- In the field of pain management and cosmetic therapy, Agnikarma procedure can be done very effectively and safely.
- Its procedures are simple and almost have no medicines for internal and external use.
- To the patients, it is very convenient and economic.
- Agnikarma procedure needed to have more scientific studies and evaluation.
- Agnikarma will become one of the most promising branches of Ayurveda in future.
